Our core purpose is to work with people and lead communities in improving their mental and physical health and wellbeing for a better life; through delivering excellent and responsive prevention, diagnosis, early intervention, treatment and care.
On the London Road Community Hospital site we currently offer one inpatient ward, Ward 1. This is an 18-bedded assessment and treatment ward for people over the age of 65 with functional mental health problems, such as depression, schizophrenia, mood disorders or anxiety. Ward 1 is a mixed sex ward with gender-specific sleeping areas with a mix of single and twin rooms which all have en-suite facilities.
Discharge planning always begins from admission and robust CPA guidelines are followed. The ward team aims to work in partnership with statutory and voluntary agencies to achieve best practice.
Every patient receives an holistic person-centred assessment by a multi-disciplinary team. All care plans are person-centred and involve the individual where possible, and all care plans are reviewed on a twice-weekly basis. All of our patients on the ward receive a twice-weekly review by the consultant at the multi-disciplinary team meeting.
Adults over the age of 65 presenting with functional mental illness or others assessed as needing the service
Patients who have been seen and assessed by a doctor or community mental health team as needing to be admitted to hospital
Reasons for admission need to be predominantly mental health and it is aimed for all patients to have been screened prior to admission for common medical problems by their GP.
On 16 January 2017 Ward 2 at LRCH temporarily closed to admissions as a result of reduced admissions. The Trust is seeking to provide a positive alternative to hospital admission for older adults with functional mental health needs.
